New healthcare delivery systems call for greater coordination of care, particularly case management for patients with complex medical needs. The advent of electronic medical records, makes it possible for physicians to access full patient records with the ability to monitor procedures, medications and doctors' appointments. Patient care navigators, also called case managers, work with physicians to ensure that their patients maintain the regimens and medications needed to improve their health. This course in community health will cover such topics as chronic diseases, culture and values in healthcare disparities, health coaching, patient care, motivational interviewing and skills needed to work with patients in treatment.
